Please note:
-- In order to accept a referral for an evaluation, we require a Referral Information Form (below). If you are a physician, clinician, or other provider referring a client/child (client may fill out the form), we request that parent/client contact us directly afterward to confirm the referral.
-- We don't do psychotherapy or counseling, we only do assessments / evaluations , although we provide a list of potential therapists in our reports , based on the issues identified.
— Presently we are scheduling evaluations about 3 months out.
---We are not able to become involved in sending evaluation reports to Social Security (but you are welcome to do, so if needed).
---We are conducting evaluations in-office / in-person only.
